Holding Hands Care & Support Services (HHCSS)

Our experienced Registered Nurses provide specialised care, including continence assessments, wound management, catheter care, medication management, diabetes care and high-intensity supports. We also collaborate with disability service providers to deliver clinical services, provide nursing expertise and strengthen clinical governance through assessments, care planning, clinical oversight and ongoing support, tailored to each organisation’s needs.

We deliver practical, nurse-led training and competency development in manual handling, medication assistance, infection prevention, catheter care, PEG feeding and complex care. We work alongside providers to strengthen workforce confidence, clinical competency and safe care practices through tailored training and ongoing education.

We provide person-centred disability support, including in-home assistance, community participation, Supported Independent Living (SIL), short-term accommodation and complex care. Our focus is on promoting independence, respecting individual choices and building meaningful community connections.

Our RN-led approach brings together compassionate support, clinical expertise and professional oversight. Whether supporting participants directly or partnering with other providers, we offer flexible, collaborative solutions through one-off clinical consultations, tailored services or ongoing clinical partnerships.
